Card for medical use in radiology.

 

Main chip is probably APEX EP20K400E embedded programmable logic device (PLD) from Altera. Card was used with 2 profi monitors in hospital. Missing compactible monitor, drivers and any info about this card (name, device it came from, anything). Tried to connect it as secondary card, Windows detects card as 3 devices - Silicon Motion LynxEM+ (probably used only as DOS device), PCI bridge and unknown device with id VEN_127F&DEV_1105&SUBSYS_905410B5&REV_00.
